We provide services at the Futebol Performance Center located at 66 Commercial Way, Unit 4B, East Providence, RI 02914. It is located in an industrial park area between Waterman Ave. and Route 44.
Do you treat athletes of all ages and sports?
We are happy to treat athletes of all abilities, ages 12 and up, and across various sports. If you have an injury or performance deficit, we are here to help you successfully achieve your goals.
What makes your rehab/performance center different from a traditional PT clinic?
Traditional PT clinics prioritize high patient volume due to insurance dictating current reimbursement rates which continue to decline locally. The result is that traditional PT clinics are forced to see more patients, often in 20-30 minute blocks and due to these volume demands, much of the treatment may be performed by support personnel or independently without supervision, and the therapist attention may be divided with multiple overlapping treatments/patients. The result is that quality of care may be impacted in this traditional model and format. Our model circumvents some of these issues by providing one on one care for 60 minute sessions, allowing improved quality and comprehensive focused care. Additionally, because our clinic has specialized performance equipment and field space, it allows you to transition seamlessly across rehab phases to allow return to sport/activity all under one roof.
Won’t paying out of pocket cost me more money?
Traditional PT clinics often see patients 2-3 times per week. With recent insurance trends of increased co-pays and high deductibles, this can often result in significant fees/charges and also significant time pressure and schedule demands. With the out of network and cash-based fee for service model we are able to see patients less frequently, because the format of longer individualized treatment sessions with one on one attention allows improved efficiency and collaboration with your care. This more comprehensive and collaborative approach allows more effective treatment results at a lower visit frequency to save you time and money.
How long does each physical therapy rehab session last?
Each physical therapy/rehab session includes 60 mins. of undivided attention from a specialized sports physical therapist focused solely on your recovery, specific goals, and performance.
How can I determine if you treat my injury?
We treat a diverse mix of acute and chronic athletic injuries, including extensive experience with post-operative care. For a more detailed list of injuries we commonly treat, please click here. We are unable to perform sports concussion and pelvic floor rehabilitation services at this facility and location out of respect for your privacy and optimal rehab conditions for a successful outcome with these conditions.
Do I need a physician or medical provider’s order to receive physical therapy services?
No, Rhode Island is a direct access state which means that certain orthopedic conditions can be treated without a referral from a specific medical provider for up to 90 days from the start of treatment. This may save you time and money to start feeling better faster! For more specific information on the Rhode Island state laws and policies on this issue please click here. However, we encourage patients to see a provider to obtain imaging, testing, and medications which may improve diagnosis and treatment outcomes. We have established working relationships with many local physicians/provider specialists and can help you make sound decisions for your care when needed.
Do you accept insurance?
No, we are an out of network and cash-based physical therapy fee-for-service provider. This model allows us to ensure the highest quality care through one on one focused, one hour treatments with time and energy focused solely on you the patient, avoiding other distractions and time-consuming administrative burdens related to traditional insurance-based care. Many patients will be reimbursed by their insurance plan and we provide a “superbill” for services that can be used to submit to your insurer for reimbursement. We do NOT perform insurance verification or deal directly with insurers, so it is up to you to confirm benefits and coverage with your individual plan or through your employer. Prospective patients are advised to call the number on your insurance card and ask, “What are my out-of-network physical therapy benefits?” PPO plans typically offer partial or full reimbursement. This linked guide may help you determine your coverage. We are able to accept flexible spending accounts (FSA) and healthcare savings account (HSA) payments toward physical therapy services.

How and when do I pay for physical therapy or sports performance services?
We accept cash or credit card, and payments are due in advance or at the time of service. Pre-paid packages are also available.
If my child is a minor, do I need to present for each session?
We request that a parent or legal guardian is present for the initial evaluation session for any children under the age of 18. After that, you will be able to sign a consent form authorizing your child to be seen without a parent or legal guardian present.
What do I need to bring with me for the initial evaluation/consultation for physical therapy?
We kindly ask that you send or ensure that your provider sends any relevant medical records in advance of your first appointment to use our time together as efficiently as possible. This also affords Catalyst staff the opportunity to thoroughly review your records in advance and prepare for our session together. You may also bring additional records and information in hand to the first appointment, but please know that the time it takes to review this information will count toward your allotted session time.
How should I dress for the initial evaluation consultation for physical therapy?
If you are being treated for a lower extremity injury, we ask you to have shorts to allow us to inspect the affected area and perform treatment to that region without limitations due to tight fitting or restrictive clothing. If you are being treated for an upper body or upper extremity injury we ask that you wear a loose fitting t-shirt, halter top, or sports bra to visually inspect, assess, and treat the affected region.
